NOTICE TO BIDDERS

 

Notice is hereby given that sealed Bids will be received for the following project:

 

Floor – Oak Lawn Patriot Station

 

Provide all labor, equipment, and materials necessary to remove and replace the floor at the Village of Oak Lawn Patriot Station located at 5100 West Museum Drive.

 

Sealed Bid Proposals will be received up to the hour of 10:00 A.M. (CST) on Wednesday, October 17, 2018, in the office of the Village Clerk in the Municipal Center located at 9446 S. Raymond Avenue, Oak Lawn, IL 60453. All sealed bid proposals will be publicly opened and read at 10:00 A.M. on the same day in the Village Clerk’s office at the Municipal Center.

 

id proposals must be submitted on the forms provided.

 

CONTRACTORS MUST ATTEND A MANDATORY PRE-BID CONFERENCE at the Village of Oak Lawn Patriot Station located at 5100 W. Museum Drive, Oak Lawn, Illinois at 10:00 A.M. ON WEDNESDAY, October 10,  2018 to discuss the Bidding Documents. CONTRACTORS WHO DO NOT ATTEND THE PRE-BID CONFERENCE WILL BE DISQUALIFIED FROM THE BIDDING PROCESS.

 

The Contractor will be required to pay no less than the Illinois Prevailing Wage Rates (as recorded by the Illinois Prevailing Wage Act – 820 ILCS 130 and in Springfield, IL) on this Project as established by the U.S Department of Labor and must comply with the equal opportunity provisions set forth in the Contract Documents. The Contractor also shall comply with all Federal, State, and Local regulations. The Village of Oak Lawn reserves the right to determine the low, responsive, responsible bidder, to wave irregularities, and to reject any or all bids.

Bid Specifications

 

Floor – Oak Lawn Patriot Train Station

 

  • Tear out of existing floor tile
  • All Concrete Floor preparation
  • Replace with Full Flake Epoxy 3 colors
  • 20 mil thickness
  • Floor must have a weather resistant top coat
  • Any control joints must be filled
  • Any Transition steps by grates and thresholds must be built up
  • Integrated cove must be 4”
  • Floor will have a 4 to 6 foot logo (Provided by Village)
  • Must have a 1 year warranty on all materials and labor
  • Contractor must meet all Village of Oak Lawn Insurance requirements (See Attached)
  • Contractor must provide Certificate of Payroll
  • Contractor must complete job during 2rd shift (3:00 P.M. – 11:00 P.M.)
  • Job to be completed by December 31, 2018.

 

Three (3) references must be supplied with photos of completed work.  To include:
- Name of Company, Address, City, State, Zip, Phone, Contact/Address & Photos of completed work.

VILLAGE OF OAK LAWN CONTRACTORS INSURANCE REQUIREMENTS

 

  1. Notwithstanding any terms, conditions or provisions, in any other writing between the parties, the contractor hereby agrees to effectuate the naming of the Village as an additional insured on the contractor’s insurance policies, with exception of workers’ compensation insurance.

     

  2. The policy naming the Village as an additional insured shall:

    Be an insurance policy from an A.M. Best rated A- or higher insurer, authorized to conduct business in Illinois.  The decision to accept specific insurers lies exclusively with the Village.

     

    State that the organization’s coverage shall be primary and non-contributory coverage for the Village its Board, employees and volunteers.

    The Village shall be listed as an additional insured by used endorsement CG 20 10 11 85 or equivalent.  Examples of equivalent ISO additional insured endorsements include using both CG 20 33 10 01 and CG 20 28 10 01 together.  A completed copy of the endorsement must be attached to certificate of insurance.

     

    The certificate of insurance must describe the specific services provided be the contractor (e.g., roofing, carpentry, and plumbing) that are covered by the commercial general liability poly and the umbrella policy.

     

    At the Village’s request, the contractor shall provide a copy of the declaration page of the liability and umbrella policies with a list of endorsements and forms.  If so requested, the contractor will provide a copy of the policy endorsements and forms.

     

  3. The contractor agrees to indemnify the Village for any applicable deductibles and self-insured retentions.

     

  4. Required Insurance:

    Commercial General Liability Insurance

    $1,000,000 per occurrence/$2,000,000 general and products/completed operations aggregates.  The General Aggregate shall apply on a per-project basis.

    Automobile Liability
    $1,000,000 combined single limit for owned, hired and borrowed and non-owned motor vehicles.

    Workers’ Compensation, Employers Liability
    Statutory Workers’ Compensation, Employers Liability Insurance and for all employees.

    Owners Contractors Protective Insurance
    (When required in the specifications)
    $1,000,000 per occurrence/$2,000,000 aggregate, with the Village as the named insured.
    Excess Insurance
    $5,000,000 (or higher) each Occurrence and Aggregate depending on the type and size of the project.  Excess coverage shall be on a follow-form basis.

    Bid, Performance and Labor & Materials Bonds
    If required, these bonds shall be provided by an Illinois admitted surety company, in good standing.

    Builders Risk Insurance or Installation Floater
    Consult your insurance advisor concerning the (1) need and (2) provider of these coverages.

     

  5. Contractor acknowledges that failure to obtain such insurance on behalf of the Village constitutes a material breach of contract and subjects it to liability for damages, indemnification and all other legal remedies available to the Village.The contractor is to provide the Village with a certificate of insurance, evidencing the above requirements have been met, prior to the commencement of work or use of facilities.
